양성자치료 관련 논문 - 2차암 발생

어제 읽은 재미난 논문 하나! 

Incidence of Second Malignancies Among Patients Treated With Proton Versus Photon Radiation

간단히 정리해보면 아래와 같다.

일반 방사선치료를 받으면 2차암이 보통 8%정도 발생한다

양성자치료를 받으면 정상조직이 1/2, 1/3로 줄어든다

MGH에서 5400명 치료했는데 (1973-2001)

eye 치료, 외국인(571), meta 등을 제외했더니 1407명 정도였고, 

MGH FU가 없는 383명도 제외 ==> 1034명 : 2007년까지 followup.

1034명을 NCI SEER data (28% US cover)와 match up.

221,000명의 nonmeta x-ray 치료환자 발견, LLUMC 때문에 LA환자는 제외, IRB 통과

1034명중에 1SEER data와 match된 환자는 558명(54%)

33% Prostate, 32% CNS, 8% musculoskelatal, 3% GI, 0.4% Lung, 0.2% Lumphoma, 44 PED

unmatched rare cancer (339 H&N, 291 base of skull, 93 NPX)

모든 환자에 대한 Median FU 6.7 (양성자)  vs 6.0 (X-ray), age 59 yr

결과적으로 보니, 2차암의 발생 빈도는 

5.2% (양성자) vs 7.5% (X-ray)였고, unmatched 양성자 환자 중에서는 3.8% 였다. 

이것을 환산하면 6.9 vs 10.3 cancers per 1000 person-year 이다.

In conclusion, after we adjusted for known prognostic factors, there was a lower incidence of second cancers among cancer patients treated with proton radiation compared with patients treated

with photon radiation. Because second cancers can appear many years after the initial radiation therapy, longer follow-up of these patients is needed. Given the limitations of the study, the reduced

second tumor rate in the proton cohort that we observed should be viewed as hypothesis generating. Nevertheless, these findings are reassuring that the risk of second tumors was at least not increased when using protons compared with photons, but leave open the question as to whether proton radiation therapy decreases rates of second tumors in radiation-treated cohorts
